POSITION: Physical Therapist DATE POSTED: August 1, 2008

REPORTS TO: Lead Therapist STATUS: FT,
temporary (3 – 4 month duration)
SUMMARY OF RESPONSIBILITIES:
· Evaluate and assesses patients for who physical therapy is necessary.
· Formulate treatment plans and goals for physical therapy patients.
· Complete all documentation related to patient care activities.
· Supervise physical therapy care provided by aides, assistants, or students.
REQUIRED QUALIFICATIONS:
- Appropriate degree from an accredited Physical Therapy program
- Two years experience as a licensed physical therapist
- Thorough knowledge of physical therapy principles and practices
- Ability to prioritize, work independently, and communicate (orally and written) effectively with patients, families, staff and physicians
- Proficiency with Windows based applications
