• Appropriate referral information
• Auto/Liability Insurance Information
• Workers' Compensation Insurance, Information w/ Claim #
• Co-Payment (Cash, Check, Mastercard, Visa
or Discover)
The appropriate referrals, confirmations, and pre-authorizations must be
provided, or your appointment will have to be rescheduled. Thank
